최첨단 유전자 검색 등 제공 “한국 정부와 일할 수 있길 기대”

한국파스퇴르연구소(소장 자바라 박사)가 최근 중동호흡기증후군(이하 메르스) 바이러스의 확산을 막으려는 정부의 노력에 적극 도우려는 입장을 내놔 눈길을 끈다.
자바라 박사는 일문일답을 통해 자신의 견해와 이해를 전하면서 한국파스퇴르연구소의 입장을 밝혔다.

현재 한국에서 메르스(MERS) 바이러스 출현이 놀랄만한 일인가?

한국에서 MERS 발병은 100년 전 비해, 전 세계적으로 수많은 여행자들이 이동하거나 국가 간의 무역거래가 증가하는 상황을 고려하면 그리 놀랄만한 일은 아니다.
하지만 모든 정부기관이 한국에서 바이러스 유행의 심각성을 정확히 인지, 분석하면서 해결할 수 있는지 시험대에 오르게 됐다.
이는 수동적인 모니터링 시스템과 달리, 능동적인 현장과 봉쇄상황을 설정해야 한다는 뜻이다.
즉 “적극적인 군사 작전”과 같은 상황으로 대처해야한다.
지금까지 상황을 들여다보면, 우리는 한국질병관리본부가 이번 최악의 바이러스 출현 사태에 대처하는 능력과 확인된 케이스에 어떻게 방역활동을 하는지 엿볼 수 있다.

우리가 MERS 바이러스에 아는 것은 무엇인가?

사실 이 바이러스에 대해선 많은 정보가 없다.
불과 2~3년 동안의 연구를 바탕으로 메르스 바이러스의 생물학적 특성을 파악하기엔 충분하지 않다.
하지만 바이러스 학자에게는 어떤 면이 새롭고 다른지 다양한 측면의 가설을 제공할 수 있다.
한국에서 메르스 확진 판정 수를 근거로 따져보면 상당히 높은 전염성을 갖고 있다.
그러나 의료보건시설에서 대다수의 환자가 메르스 바이러스 감염자로 진단 받기 전, 그리고, 적절한 격리 조치가 전에 병원체에 노출됐다.

중동지역에선 한국과 같은 처지에서 병원 내 바이러스가 출현 했을 당시, 적절한 격리 조치를 했으며 이후 병원성 감염 사태가 중단됐다는 사실을 알고 있다.
하지만 한국의 사정은 다르다.
메르스에 감염된 한국인에게서 채취한 바이러스의 유전체 서열정보를 알 수 없는 상황이다.
게다가 다른 나라에서 발생한 샘플과 비교할 수 없는데다 아마 한국인에게 전파되는 바이러스는 변형된 것일 수 있다고 추측해볼 수 있다.
현재로선 아직 메르스 바이러스에 관한 연구가 충분하지 않아 제대로 된 치료제를 개발하지 못했다.
이에, 더 많은 노력이 필요할 것이며 특별히 치료제 개발이 절실히 필요하다.

이번 메르스 사태에 한국파스퇴르연구소는 어떤 도움을 줄 수 있는가?

언제라도 한국 정부로부터 도움의 요청을 받는다면, 연구소 생물안전3등급 실험실(BSL-3)에 있는 Screening Technology platform 팀을 구성, 약물 탐색을 하기 위해 살아있는 바이러스를 사용해 초고속·대용량 약물 스크리닝을 진행할 준비가 돼있다.

우리는 미국 식약청에서 이미 승인된 약물 군을 스크리닝 해, 잠재적으로 감염된 환자를 치료할 수 있는 오프 레벨 사용 가능한 유효 약물을 찾아내는 것을 시작할 수 있다.

한국파스퇴르연구소는 호흡기 바이러스 연구를 전문으로 하는 연구팀을 운영하고 있으며, 파스퇴르 국제 네트워크를 통해 전문가이나 특히 홍콩파스퇴르연구소의 Malik Peiris 교수의 도움을 받을 수 있다.
또 한국파스퇴르연구소는 파리 파스퇴르 연구소에 최첨단 유전자 검색과 생물정보학 기술을 활용할 수 있다.
한국파스퇴르연구소는 한국 정부, WHO, 다른 국제 파트너 기관들과 함께 일할 것을 기대하고 있다.

전 세계적으로 바이러스 출현에 대한 대응책은 마련돼 있는가?

2014년 출간된 Combinatorial Chemistry and High Throughput Screening 사설에서, 하킴자바라소장(당시 미국 뉴욕시 메모리얼 슬론 케터링 암센터의 소속)은 다가오는 세계 공중보건위기 상황에서, 현존하거나 앞으로 새롭게 진단될 질병을 퇴치할 치료제 공급의 감소와 한계란 각각의 주제로 기고했다.

두 가지 예를 들면 그가 주 골자로 선택한 전염병으로는 지금까지 치료제가 없는 에볼라와 최근 발생한 메르스 코로나 바이러스다.
메르스 코로나 바이러스는 이미 미국만큼 멀리 떨어진 사우디아라비아에서 한국까지 전파돼왔다.

어느 누구도 이후에 이 바이러스가 어떻게 혹은 어느 곳에서 발병 될 것인지 예측할 수 없다. 정부, 각 나라의 질병관리본부와 세계보건기구 WHO는 바이러스가 발병할 수도 있거나 발병하지 않을 수 있는 상황에서, 24시간 감시하는 것은 불가능한 일이다.
이를 퇴치할 치료제도 아직 개발되지 않았다.

오늘날 세계는 감염성 질병 출현에 대한 대응책이 충분히 마련되지 않고 있다.
한국정부는 결코 쉬운 일이 아닌 이런 사태에 대한 철저한 관리를 위한 노력에 총력을 기우려야 한다.

Hakim Djaballah, CEO of Institut Pasteur Korea, responds to comments made on the current MERS-CoV situation in Korea.

Institut Pasteur Korea (IPK) is deeply saddened by the unfolding events caused by this recent MERS-CoV outbreak. IPK is fully supportive of all ongoing efforts to contain the outbreak by the Korean government. We provide below some clarification on Dr. Djaballah's published comments and understanding of the MERS-CoV situation.

Is this outbreak surprising in this part of the World?

The current outbreak in Korea is not surprising considering that globally we are more mobile as travelers than a century ago, more and more trades are happening between far away countries. It provides, however, the ultimate stress test-opportunity for any government to discover, confirm, and deal with such outbreaks; it is completely different from a passive monitoring, to an active field assessment and containment situations - this becomes more like an "active military field operation". The Korean CDC has, thus far, clearly demonstrated their response capabilities to this unfortunate outbreak and their sharing of information about the cases as they are identified.

What do we know about the virus?

We do not know a lot about this virus. Two to three years of research is not enough to learn about its biology. For virologists, it provides working hypotheses on many aspects of what is new and different about it. Based on the confirmed number of cases in Korea, it may appear to be highly contagious, but we know that the majority of the cases associated with health care facilities have resulted from exposure before MERS-CoV was diagnosed in a patient and before adequate control measures were put in place. We also know that there have been other nosocomial outbreaks in the Middle East that were stopped when adequate controls were put in place. At the same time, it is fair to speculate that perhaps this version of the virus has somehow mutated since we do not have access to sequence information for the Korean isolates to compare them with other worldwide isolates, we understand that this work is underway. Not enough research has been done to date and not enough to help us find treatment for MERS-CoV. More work needs to be done and especially on drug discovery efforts.

What can our institute do to help?

If and when requested by the Korean government, we are ready to deploy our screening technology platform located in our Biosafety Level 3 laboratory to rapidly develop a high content assay using live virus to screen for drugs. We can start with screening the panel of US FDA approved drugs for any active drugs for potential off label use to treat infected patients. We have a research laboratory dedicated to respiratory viruses and expertise on the MERS-CoV from our colleagues within the Institut Pasteur International Network, especially, Prof Malik Peiris at the HKU-Pasteur Research Pole in Hong Kong; and also access to state of art sequencing and bioinformatic capabilities at the Institut Pasteur in Paris, France. We are happy to work with the Korean government, the World Health Organization (WHO), and other international partners.

Is the World prepared for outbreaks?

In an editorial published last year in Combinatorial Chemistry and High Throughput Screening, our CEO, then at Memorial Sloan Kettering Cancer Center in New York, USA, wrote on the looming global health crisis to do with the decline and limitations of current drug arsenal to fight existing and newly diagnosed diseases citing two examples: 1) Ebola, where there is currently no approved treatment; and 2) the recently discovered MERS-CoV; this virus already travelled as far away as the USA from Saudi Arabia and it is now in Korea. No one can predict how and where the next outbreak will occur, leaving governments, their centers for disease control, and WHO in an impossible position to mount a 24 hour daily surveillance on a "virus" which may or may not arrive; and when it arrives, there is no treatment to combat it with either. The world today is not as prepared to handle infectious disease outbreaks. The Korean government should be commended for their efforts in controlling the situation, not an easy task by any means.
